Year 8 Vaccinations

The School Aged Immunisation Service is offering the Human Papillomavirus (HPV) vaccination to Year 8 students (aged 12 to 13 years) on Friday, 5^th May.

The vaccination sessions take place in schools during the school day but there are also community clinic sessions available after school and on selected weekends.

If you wish for your child to receive the vaccination, please complete the e-consent form by Tuesday, 2^nd May by clicking on the link below and following the instructions.

• Click START you will then be asked for the Unique School LEA Code which is: 145372

If this date has passed you can still complete the consent form and you will have the option to book into one of our community clinics. If your child is absent or is unable to have the vaccine on the day you will receive an email to book an alternative appointment.
