Year 9 Vaccinations

The School Aged Immunisation Service will be offering the fifth and final vaccination (2nd booster) following on from the Pre-school Booster for Tetanus, Diphtheria and Polio, and booster for MenACWY which completes this course to Year 9 students on Friday, 27th January 2023.
If you wish for your child to receive the vaccination, please complete the e-consent form by Tuesday, 24th January by clicking on the link below and following the instructions:
- Prior to completing the online E consent, if you are unsure of your child’s Meningitis ACWY and Tetanus, Diphtheria and Polio vaccination history, please contact your GP/or access NHS App for your child/ren to check as this is required in the process.

- Click START, you will then be asked for the Unique School LEA Code which is: 145372
If your child has already had one of these vaccines, you can choose the vaccine they have not yet had on the consent form. If you know your child has received both the 2nd booster and MenACWY dose since attending secondary school, you do not need to complete the consent form.
